This book addresses the critically important issue of measuring the outcomes of patient education in the management of chronic disease.  Approximately 15 diseases are covered in this theoretically-based manual, which includes evidence-based standards for patient education for each disease.  Also included are two instruments for assisting and measuring outcomes for each disease, along with methods for achieving desired outcomes.  Given the focus of the health care delivery system on cost-effective health care, Patient Self-Management of Chronic Disease will provide primary health care providers, nurse practitioners, and clinical nurse specialists with the tools they need to increase the effectiveness of their patient education programs.
